Completed application form
Valid identification (passport, drivers' license or national ID)
Tax Registration Number (TRN)
Two (2) passport size photographs
Valid license (Route, Hackney , etc.)
Two (2) written references (at least one from a Justice of the Peace, school principal, pastor or senior police officer)
Proof of address (utility bills or verification forms signed and sealed by a Justice of the Peace Verification forms are provided by JNSBL)
